All student athletes must be medically cleared to participate in any competitive sports program. The Adelson Educational Campus has arranged for medical professionals to provide these required physicals free of charge throughout the year on campus.

Students wishing to participate in sports at The Adelson Educational Campus must complete NIAA Health Forms B & D.

Form B – NIAA Pre-Participation History Form must be completed before the first day of practice for each season a student wishes to participate.

Form D – NIAA Pre-Participation Physical Evaluation Form must be completed annually, no earlier than one year prior to the first day of the month that the specific sport season begins and is good for all sport seasons within that year.

Doctor Emergency Services/EMCARE
This medical team along with Nurse Liz conducts our free sports physicals for student athletes in grades 5-12. Families will be notified of screening dates via mail or web posting. To schedule an exam for your child, make an appointment with Nurse Liz (702-515-8207) and print and sign the consent form.  Give the signed form to your child to bring at the time of the appointment.
